Many patients these days are fed up with impersonal managed healthcare and yearn for the Good Old Days when they felt they had a close personal relationship with their doctor.
In response to the growing demand for quality care, a few doctors in large cities all over the country are now beginning to limit their practices to providing exclusive "VIP" or "Concierge" medical services to a small, select group of patients who are willing to pay a flat annual fee ranging from $900-4,000/year.
In return for their annual fee, "VIP" patients receive:
- Immediate direct cell phone access to their doctor 24 hours a day, seven days a week.
- Same-day appointments.
- On-time appointments
- As much time with their doctor as they wish.
- Active involvement by their doctor in keeping them healthy.

My patients, of course, are going to wonder, "Well, what's the big deal about that other than the big annual fee? That's the way Dr. Jones has always taken care of me."
My patients have been accustomed to receiving that kind of VIP service from me for the past 30 years. I have always treated every one of my patients like a VIP, because every single one of them is a VIP to me; and I have insisted that my office maintain our same high standards despite all the pressure from the managed care companies during the past 10 years intended to force doctors to dilute and degrade the quality of care we give our patients.
The managed care debacle has deteriorated steadily, to the point that in 2001 I finally realized I had no alternative but to rid my office of all the managed care companies if I intended to maintain the high quality of care to which my patients have always been accustomed.. My patients are accustomed to paying at the time of service practically everywhere else they go (hair dresser, veterinarian, dentist, restaurants, etc.); and I am pleased that very few have expressed any objection to paying me a fair fee at the time of service, then filing for their own out-of-network benefits.
